Coordinates and directs care of an assigned group of residents during a 12- hour shift according to physician orders and facility practices. Follows physician orders and the resident’s care plan and provides accurate, timely documentation to support care delivery. Must be licensed RN or LPN.

Provides quality resident care as specified on an individualized resident care plan and assignment sheets, according to established policies, procedures, and objectives of the facility. Licensed CNA’s preferred, but able to train and certify.

This position assists in meeting the nutritional needs of residents in a long-term care facility. Duties include providing appropriate foods and fluids as indicated on diet cards and performing a variety of services related to dining.
FULL-TIME • 32 HRS PER WEEK • DAY SHIFT
To maintain the facility and grounds in good repair, in compliance with all applicable codes. Have a thorough knowledge of mechanical, electrical, plumbing, heating and ventilation systems. Adheres to Federal/State regulations and guidelines, including Resident Rights. Must be able to display tact, empathy and good judgment in dealing with varying situations involving residents, residents’ families, visitors, management, physicians and staff.
FULL-TIME • MONDAY – FRIDAY • 40 HRS PER WEEK
Coordinates all aspects of the Sheyenne Care Center computer network and communication systems, including telephone, security access, nurse call, and other electronic systems. The IT responsibility also provides employee training on use of the computer system and be responsible for developing and creating analytical reports for business office related systems. Business office responsibility is to assist the Finance Coordinator with finance reports, Account Receivable, and other business office duties.

Managing the Infection Prevention program which includes Antibiotic Stewardship, immunizations, COVID testing, COVID infections and immunization, QA of the infection prevention program, providing education related to infection prevention to all departments. Works closely with all departments in regards to infection control. Updating and writing policies and procedures related to infection control. Infection control experience preferred but not required. RN preferred, but not required.

The SMP Health System provides a high deductible health care plan with an HSA. Please see your Human Resources Specialist regarding plan benefits and enrollment criteria.
If you do not have an HSA, you might consider a Flexible Spending Account, which provides you with the opportunity through payroll deduction to set aside a portion of what would have been taxable income and deposit the money into a Personal Reimbursement Account.
A 3% contribution is provided by the SMP Health System at no cost to the employee. The 3% contribution is based on an employee’s gross earnings after one year of service by the SMP facility. To meet eligibility requirements, employees must complete 1,000 hours of service within a 12-month period, and the employee must be 21 years of age.

Paid Time Off is provided to each employee in a benefit-eligible position of at least fifteen (15) hours per week. It is an account in which all earned credit for vacation, holidays, and short-term sick time accumulates. PTO is earned at a set ratio which increases with years of service.
There are seven (7) paid holidays per year. These are built into an employee’s Paid Time Off (PTO) benefit.
Regular full-time and part-time hourly employees receive one-and-one-half (1 ½) times their regular rate of pay for working on the paid holiday.
